Summary of ServiceTBI Residential Rehab LLC is committed to the clients, families, stakeholders, and communities we serve. We work directly with the client and their treatment team to make sure client’s needs are met. Each client has a personalized treatment plan developed with input from all stakeholders and implemented by our trained staff. Stakeholders may include the family member(s), guardians, referral facility discharge planners, treating medical provider, case managers, adjustors, attorneys, and other interested parties. Progress is continually measured, and plans are often updated to ensure that our client’s needs are being met. Our residential program is designed to provide rehabilitation for clients recovering from brain injury, mental health, and physical disability. TBI Residential Rehab provides a structured living environment that emphasizes functional gains and provides clear, objective outcome measures. The Residential Program includes: Safe, Supportive Living Environment Supervision Protection Monitoring of Personal Care Special Diet Accommodations Monitoring and Assistance with ADLs Medication Reminders Drug Testing Mental Health Services as Needed Behavior Monitoring Assistance with Money Management and Budgeting Assistance with Meal Preparation and Planning Internal Service Coordination Comprehensive Individual Care Plans Transportation to Appointments within a 10-mile Radius Community Outings and Re-Integration Recreational Activities Semi-Independent Apartment Program includes: Behavior support services Staff available by phone 24 hours a day 24 hour on-call access to administration Nursing on site (as needed) Transportation up to 10 miles for appointments, home visits, etc Recreational Activities Room and board Services Available by Referral to Community Resource Physician Services Psychiatric Services Vocational Services Rehab Services Other services as needed.

Staff ExperienceTBI Residential Rehab staff are required to maintain training in all areas of state licensure requirements, which can also include experience with brain injury, mental health and physical disabilities In addition, staff participate in annual job specific training in Recipient Rights, Blood Borne Pathogens, Person Centered Planning, Grievance & Appeals, Cultural Diversity, Limited English Proficiency, CPR/First Aid, HIPAA, Nutrition, Confidentiality, Fire Safety, Health and Safety, Behavior Modification, and Medication Monitoring, which includes distribution and storage of medication only allowable following the written orders of a licensed physician for the purpose of maintaining health and safety.

Population(s) ServedTBI Residential Rehab provides services for client’s recovering from traumatic brain injury, mental health challenges and physical disabilities, 18 years of age or higher.

SettingsTBI Residential Rehab operates residential services in a State Licensed AFC small (6 bed) group home located in the Rochester Hills, Michigan. In addition, TBI Residential Rehab leases apartments in the Rochester Hills community to provide Semi-Independent Living Services. The setting for additional ancillary services depends on the identified needs and goals of the person served.

Referral ProcessCalls are made by potential clients and stakeholders to the administrator and/or licensee . referrals can be made to Nina Lang, Licensee at (248) 361-1389 Cell or (248) 355-5800 Office and Vicky Altadonna, Administrator at (248)238-4046.

Who can make a referralSelf-referrals are accepted as well as referrals from family members, guardians, physicians, case managers, automotive insurance carriers, or health and human service providers such as therapists and/or hospital social workers, discharge planners, private pay, worker’s compensation, and other funding sources.

Referral ProcessCommunication is made to the Administrator or Licensee and client is screened for appropriateness based on level of care and needs. A tour is scheduled with the client and other interested parties. If client is appropriate for services, a formal intake is scheduled.

Documents or information the consumer needs in order to enrollA potential client will need a physician’s order, operator’s license or state ID card, health care appraisal, a negative TB test or chest X-Ray, social security card, medical insurance cards, confirmed payor source, a diagnosis of a brain injury, mental illness or physical handicap related to an auto injury.

Funding SourceWe accept auto insurance, worker’s comp, and private pay. A fee schedule and detailed description of services is available upon request. A change in fee schedule is communicated to all funding sources through a revised bill reflecting changes with a 30-day notice. A Residential Care Agreement is updated with revised charges given to the client and/or guardian.

Cost to ConsumerFees for service are covered by either auto insurance, worker’s compensation, or private pay. Service rates are based on the needs of the client.
